How do I obtain an accident report by mail or fax?
You may also obtain an accident report by mail or fax. If requesting an accident report by mail or fax, there is a handling fee of $8.55.
If a crash report card was issued by the officer, please provide the agency tracking number. If you request a report and do not have an orange or white crash report card from the officer, you will need to provide the following information:
- Name of one of the drivers involved (not the insured/owner’s name).
- Date of the accident.
- (Optional) – Location of the accident.
**Those parties not involved will still need to provide the required documentation as listed above.
